INSURANCE & FEES
Insurance, fees & the Good Faith Estimate
What we accept, what you'll actually pay, and what the law requires us to tell you before you start.
Plans accepted
Full list of accepted plans to be supplied by the client.
What you'll actually pay
Copay — a fixed amount you pay per session, set by your plan.
Deductible — an amount you pay out of pocket before your plan starts covering sessions.
Coinsurance — a percentage of the session cost you pay after your deductible is met.
Self-pay rates
Publish real self-pay rates here — practices that hide fees lose the self-pay client at the first click.
Good Faith Estimate
Under the federal No Surprises Act, clients who are uninsured or choosing not to use insurance have the right to receive a Good Faith Estimate of expected charges in writing before care begins.
Cancellation policy
State the cancellation window and fee plainly here, including that insurance never covers a missed appointment.
Sliding scale
Confirm whether a sliding scale is offered, and how to ask for it without an awkward conversation.
Verify my benefits
Tell us your plan and we'll confirm your coverage before your first session.
This form is secure. Please don't include detailed health information here — save it for your first session.
